Booking Form

The signed booking form should be returned within 5 days to secure the entertainment, date, and time of your event. The non-refundable booking fee of $50 should be paid within 5 days to secure entertainment. After you click on the Submit button on the second page you will be taken to the Payment Page to pay the booking fee.  Remember your event is not booked unless you pay the booking fee.
